Medical Home Demonstration Project

The League and 10 health center members have partnered with the Massachusetts' Executive Office of Health and Human Services (EOHHS) to demonstrate sustainable primary care practice changes for health quality, access and efficiency. The partnership was one of five sites funded by the Commonwealth Fund's national Safety Net Medical Home Initiative in the spring of 2009. The League, as Medical Home Facilitator, will provide technical assistance, guiding both the process and quality improvement work at health centers over the course of this four-year initiative.
