Diabetes Management
The prevalence of diabetes in Massachusetts more than doubled, from 3.8% to 8.9%, across all ages and ethnicities over the past 20 years. The Centers for Disease Control and Prevention (CDC) estimates that the number of Americans with diabetes will range from 1 in 3 to 1 in 5 by 2050. In Massachusetts, an estimated 33,400 adults are diagnosed with diabetes each year.
The prevalence of diabetes is not equitably distributed across race/ ethnicity categories. Nationally, American Indian/Alaska Native, Black, Native Hawaiian, Asian, and Hispanic residents all experience higher prevalence of diabetes than white counterparts. Community health center patients in Massachusetts also experience disparities in rates of uncontrolled diabetes.
Diabetes Training and Technical Assistance
To support health centers in improving patient health outcomes and ensuring equity in diabetes care for all Massachusetts residents, the Mass League is pleased to offer a variety of technical assistance opportunities and resources.
Utilizing Uniform Data System (UDS) data, Mass League staff conduct data analysis and provide health centers with health equity score cards indicating differences in uncontrolled diabetes based on patient race and ethnicity. This is a portfolio of resources intended for community health center staff committed to improving outcomes for patients with diabetes. These resources specifically focus on interventions that a health center can readily implement into practice. Based on the National Association of Community Health Centers (NACHC) framework, the toolkit structures interventions into four categories: Patient Activation and Behavior Change, Reducing Health Inequities, Optimizing Teams, and Health Systems and Infrastructure.
This tool, often used in learning collaboratives, assists health centers in identifying research-based drivers of positive change in patient outcomes. The tool suggests changes that health centers can test in PDSAs in order to support patients and providers in diabetes management.
