The Health Center Controlled Network (HCCN) works closely with Azara Healthcare, a leading provider of data-driven reporting and analytics for the community health marketplace, to continuously develop an advanced reporting tool known as the Data Reporting and Visualization System (DRVS). In addition to directing DRVS utilization for reporting, the HCCN offers preparatory training and targeted assistance for federal regulatory compliance, such as the Uniform Data System (UDS) annual report.  The HCCN also assists with training inexperienced users, building custom dashboards and reports, and escalating support tickets to quicker resolutions.

Reporting Training

What Is the DRVS?

The DRVS is a centralized data reporting and analytics solution for community health centers (CHCs), primary care associations (PCAs), and HCCNs that is used to facilitate care transformation, drive quality improvement, aid in cost reduction, and simplify mandated reporting. The DRVS extracts data from aligned electronic health records nightly, storing it within a secure central repository where centers can produce a variety of quality reports on key performance indicators. The DRVS was originally developed in 2009 by the Massachusetts League of Community Health Centers in partnership with Azara Healthcare.

UDS

The DRVS compiles aggregate clinical data into templated measures and reports based on common federal regulatory requirements, such as the Health Resources and Services Administration’s mandated UDS reporting. In addition to DRVS support, the HCCN provides other opportunities for focused training and technical assistance with federally regulated data reporting.

  • The UDS is a comprehensive dataset required by the Health Resources and Services Administration to assess the operations and performance of CHCs. It encompasses a wide range of information, such as patient demographics, services provided, staffing, clinical outcomes, utilization rates, costs, and revenues. UDS data is collected from CHC programs, including those funded under Section 330 of the Public Health Service Act and Look-Alike programs.
  • The HCCN provides all Federally Qualified Health Centers with an annual UDS Trends Analysis Report Package. The report package includes UDS trending information for the percent of change from previous reporting years for the 18 clinical quality measures (CQMs).
